Incident Details
What happened
Include who was involved, what occurred, when, where, and any relevant background. Include staff actions taken.

Under 21: If this incident involves abuse or neglect, a mandatory report to the local public children services agency is required.

Physical Intervention, Restraint & Seclusion
Critical
Why this is asked separately

A physical intervention, restraint, or seclusion by staff may constitute a separate unapproved behavioral support MUI under OAC 5123-17-02 — independent of whether the underlying incident meets MUI criteria. This is one of the most commonly missed classification pathways.

Individual's Condition
Medical Evaluation
Why this matters

Medical clearance by a nurse or physician is a significant factor. If on-site medical staff cleared the individual with no significant findings, that directly affects whether MUI criteria are met for injury-based incident types.

Client Notecard Generator
What this does

Paste in an individual's ISP, BIP, or behavior support plan and CertaIQ generates a one-page quick reference card — triggers, de-escalation strategies, who to call in a crisis, key behavioral notes, and emergency contacts. Print it and keep it with the individual's file.

Upload a PDF or Word document, or paste the text directly. CertaIQ will extract triggers, de-escalation strategies, crisis steps, authorized interventions, key contacts, and more.

Policy Intelligence
What this does

Upload or paste your board's supplemental policies, procedures, or local guidelines. Then ask any compliance question and CertaIQ will answer based on your specific board policies layered on top of the universal Ohio DODD regulatory framework. This is your board's custom knowledge base.

This can be your board's MUI supplemental policy, notification procedures, escalation guidelines, or any other compliance-related document. The more specific, the better the answers.
— or paste policy text directly —
Ask anything about your board's policies or how they interact with the Ohio DODD regulatory framework.

ODEW Annual Reporting
ODEW Annual Reporting Requirement

Under OAC 3301-35-15(K), all restraint and seclusion incidents for individuals ages 5-21 in school settings must be reported annually to the Ohio Department of Education and Workforce. Log each incident here and export when ready for submission. Incidents are stored for this browser session.

Note: Do not enter individually identifiable information (full names, DOBs) until Phase 2 secure storage is implemented. Use initials or case reference numbers for now.

Grievance & Rights Tracking
What this does

Log and track individual grievances and rights code concerns. Under ORC 5123.62, individuals with developmental disabilities have enumerated rights — violations may constitute a Category A MUI. Log grievances here, track their status, and export for administrative review or audit purposes.

Note: Use initials or reference numbers rather than full names until Phase 2 secure storage is implemented. Log is stored for this browser session only — export before closing.
